Apothecary Pharma is recalling 22 vials of its tirzepatide, the name under which the US-based compounding pharmacy sells its compounded weight-loss medication.

The recall, issued on September 24 by the Food and Drug Administration (FDA), concerns specific vials prepared by the pharmacy over potential contamination—not the brand-name drugs Mounjaro or Zepbound. Although tirzepatide is the active ingredient in both medications, Apothecary Pharma’s compounded product is separate from the commercially manufactured versions sold under those brands; therefore, the recall does not apply to Mounjaro or Zepbound manufactured by their respective pharmaceutical companies.

Both recalls are classified as Class II by the FDA, meaning use of the products could cause temporary or medically reversible health consequences, while severe consequences are considered unlikely. The pharmacy had previously received an FDA warning related to sterility assurance.

Consumers who use compounded tirzepatide should check the manufacturer and lot number on their vial, especially if they obtained it through a telehealth provider.

The recalled products are labeled 18 mg/mL in 4 mL multidose vials. The affected lot numbers are:
